Chemouncleolysis for the Treatment of Hermiated Lmbar Intervertebral Disc


Abstract
A herniated lumbar intervertebral disc has been the most common cause of low back pain and sciatica since Mixter and Barr reported it in 1934. Ten to twenty percent of herniated lumbar disc patients who did not respond well with conservative treatment were received surgical treatment. Recently. since Smith introduced chemonucleolysis in 1964, it has been widely applied after approval at F.D.A.
Chemonucleolysis was carried out in 14 cases at¢¥ Dept. of O.S. at Kosin Medical College from Nov. 1984 to Oct. 1985 and following re-suits were obtained.
1) The ratio between male and female was equal and the most common age group of this analysis was 2nd decade.
2) The most common site of herniation is L4-5(86%).
3) Average period of hospitalization was 7 days.
4) Among the 14 patients who were on discharge, excellent or good results were obtained in 93% and there was no poor result.
5) There was no serious complication such as anaphylaxis, paraplegia and death. As a minor adverse reactions, there were 9 cases of back muscle spasm & low back pain and 1 case of nerve root irritation sign by needle.
But all adverse reactions were regressed spontaneously.
6 ) For the poor economic state of the patient, one vial of Chymopapain was divided and injected to the two patients simultaneously.
7 ) The epidural injection of steroid with lidocaine for the back muscle spasm and severe back pain was ineffective.
